Personal Health Care

Nouvelle HHC offers personal care, household support, and companionship for those who require assistance. One of our core beliefs is everyone should remain independent in the comfort of their homes. Nouvelle HHC caregivers are trained to provide care to those who prefer a higher level of focused care. Nouvelle also offer services for patients who reside in assisted living, or independent living community.

Personal-Care Assistance

    • Dressing and grooming

    • Bathing and toileting

    • Brushing teeth, Denture care

    • Toileting and incontinence care

    • Walking and moving

Household Support Services

    • Trash removal

    • Light housekeeping

    • Emotional support and social interaction to combat loneliness

    • Supervision and safety monitoring

    • Respite care provides temporary relief for primary family caregiver

Medical Equipment

    • Inspecting medical equipment is functioning safely

    • Ordering and tracking necessary supplies

Transportation

  • Arranging or providing transportation to medical appointments and errands

Remote Monitoring

  • Leveraging technology to monitor the patients’ vital signs and condition remotely , providing quicker intervention.

Skilled Nursing

From routine medical support to comprehensive post surgical care or rehabilitative care. Nouvelle HHC offers our patients the comfort of recovering at home .Each of Nouvelles’ fully licensed and certified skilled nurses are uniquely matched with a patient based on their prior experiences ensuring our patient receives the attention and care they have come to expect from Nouvelle HHC. Having a balanced staff of Doctors (PAs), registered nurses (RNs) and licensed practical nurses (LPNs) with crucial experience across a variety of illnesses, injuries, and disabilities ensures Nouvelle Home Health Care deliveries first in class short-term or long-term medical care.

Skilled Nursing Services Provided 

      • Wound Care and Dressing changes ( complex wounds and VAC therapy)

      • Medication administration (Injections, IV therapy, Medication management and setup) .

      • Chronic Disease management (COPD, CHF, Diabetes), including monitoring and patient education.

      • Monitoring of vitals and unstable health status

      • Catheter care, Colostomy care, and enteral (tube) feeding assistance.

      • Pain management

      • Patient and caregiver education on disease, medication and self-care techniques

      Nutritional Support
     
    •  Dietary assessment and counseling by a registered dietitian.
    • Specialized meal planning for conditions like diabetes or heart disease

    Social Service

       

        • Counseling and support for emotional and social needs related to illness or recovery .

        • Connecting patients and families with community resources (Finical, food, equipment).
